Switch toolkit to use Docker Machine

Urgent update from b2d to docker toolbox / docker machine.

Shouldn't be too much work.

main commands are,

• docker-machine start default - start the VM
• docker-machine env - set env vars

and that's it. We could run these ourselves but is risky - perhaps just better output help msg for now.
