DDUw was re-scanning already-resolved documentation issues when spec-librarian follow-up PRs omitted explicit closes/fixes/resolves issue references. This change improves matching in Step 1c and adds source-side guidance so future spec-fix PRs are linkable by default.

DDUw Step 1c: add fallback match heuristic
- Extended .github/workflows/daily-doc-updater.md for closed as completed issues:
  - If no explicit issue-reference PR is found, infer package from issue context.
  - Search merged PRs in a tight closure window (±60 minutes) that touched pkg/<package>/README.md.
  - Treat issue as addressed when that PR’s README change fully resolves the reported gap.
Spec-librarian output guidance: require explicit closure references
- Updated .github/workflows/spec-librarian.md action items to instruct fix PR authors/agents to include:
  - Closes #<issue> (or Fixes / Resolves) in the PR description.
Heuristic example added to prompt
